Please note:

OK Kosher’s registered Certification Marks are internationally registered trademarks. Unauthorized use of the OK Kosher symbol or any other of OK Kosher’s registered Certification Marks ([icon name=logo-c-k] or [icon name=logo-heart-k]), beyond termination, constitutes trademark infringement and may be subject to a product withdrawal, litigation, and/or civil fines by governmental agencies..

Unused labels bearing the Certification Marks must be destroyed in the presence of an OK Kosher field representative or have the Certification Marks permanently obliterated from the labels.

Your company will be held responsible for charges for certification prior to the termination date.

I understand that my certification will be terminated as of the expiration date of my Certification Agreement. I confirm that OK Kosher’s Certification Marks ( OK , [icon name=logo-c-k] or [icon name=logo-heart-k]) will no longer be on all my products and promotional materials (including product/packaging images containing OK symbol from all online retail or advertising etc.). I also understand that once I terminate my certification, I will be required to pay for a new certification setup should I choose to reinstate my certification after 30 days of termination.
